Patrick, Elizabeth Church, Hilltop and Wesley Village Campuses for people who need help to recover following surgery, a stroke or for other reasons. Therapy programs are individualized and designed to speed recovery and return patients to their highest level of functional ability. Short-term rehabilitation beds are available for patients who require therapy services for:
An individualized care plan with measurable goals is developed for every short-term rehabilitation patient. Therapy services are provided at a level appropriate for each patient’s needs and abilities.
Social services staff coordinate with nursing and therapy staff to prepare patients for their return home. Discharge planning includes a consultation with the patient’s primary care provider and family members, as well as a home evaluation, during which therapists monitor how well the patient can move about and manage his or her daily routine at home. Home health services are initiated as needed prior to discharge.
United Methodist Homes was the first to offer short-term rehabilitation in our community. Our team has more than 50 years of combined experience, and provides physical, occupational and speech/language therapy, including swallowing therapy. Nursing and social services staff work as a team with therapists to help patients achieve the highest level of functional ability in order to promote a safe and independent return home.
