We are excited to share an important addition to our continuum of care at UMH Wesley Village: our new Outpatient & In-Home Rehabilitation Program, Wesley Village Rehab Solutions. It is designed to support recovery, restore independence, and help individuals return to the life they love as safely and confidently as possible.
Whether recovering from surgery, illness, injury, or a hospital stay, the transition back home can feel overwhelming. Our rehabilitation team is here to make that journey smoother, more supportive, and centered around each individual’s goals. Wesley Village Rehab Solutions is an option for those who visit our campus for rehab, live on our campus in independent living, or return home in the community directly from the hospital.

At Wesley Village Rehab Solutions, rehabilitation is not one-size-fits-all. Each patient receives a personalized care plan developed by our interdisciplinary team of skilled professionals. Through collaboration between therapy staff, nursing, physicians, and support services, we focus on measurable progress and meaningful outcomes.
